A Simple Question

The other day I was filling out some legal documents where I had to write in my first, middle, and last name, and it brought me back to the good old days of standardized tests and scantrons. Then (don't ask me how), I thought to myself, what does Heidi do on these? She's got five or six middle names, and I'm sure that neither her first or last name fits in the slots that they give you. So, Heidi, please enlighten me.
